•  A medical referral is not necessary.
• Appointments are available Monday through Friday from 7:00 a.m. - 6:45 p.m.
• Phone 492-4752
• Inform the receptionist if your injury is related to a motor vehicle accident or a work place accident. 
• Things to remember:
  • Bring a comfortable pair of shorts, t-shirt and Alberta Health Care card to your first appointment.
  • Arrive 10 - 15 minutes prior to the time of your first appointment to permit registration and completion of any necessary forms, or you can pre-register by printing and completeing the registration forms here.
