If you are a new patient of the clinic and have an appointment booked with a Physical Therapist please print and complete both of the forms below, and either fax them to 492-1637 or bring them with you to your scheduled appointment.

If you have previously seen a Physical Therapist at the Glen Sather Clinic, but are coming in for an assessment of a different body part, you only need to complete the " Physical Therapy Medical History Form," unless you have had a change in address, emergency contact, etc.

If you are a new patient booked with a Sports Medicine Physician you need only complete the "Patient Registration Form."
